Renaming a file causes duplicate definitions for languages created with NBL/index
When a file is renamed, the definitions from the old file name stay in the index, causing duplicate definitions in the file with the new file name.

r25759, but still needs to be applied to the unstable branch.
Also needs to be applied to the NBL editor.
